We have had a similar issue. Our puppy does great if we staya on a schedule. Monday through Friday when we work he is walked 3 times a day, before his morning food, as soon as we get home, and before bed. He generally poops all three times and then does not have accidents in between.
For some reason on the weekends he may have an accident. It is like he doesn't know how to ask to go out. It is getting better as he gets olders.
We had a trainer with a previous puppy who told us to feed him in the crate and then take him out 15 minutes later to potty. If he didn't go he went back in to the crate. If he only did one job, he got supervised play for 15 minutes and then was taken back out, every 15 minutes until he went!
